| Adult Case Management
PURPOSE: To assist individuals and families of individuals, who suffer from a severe and persistent mental disorder to remain in their home and community. A case manager’s role includes assisting the individual by helping remove barriers to recovery, and assisting the individual in obtaining resources and services that are necessary to ensure continued individual growth.
PROCEDURE: Case managers and clients assess the individual’s strengths, needs, skills, supports, and resources. This information is brought to the treatment team and integrated into a comprehensive treatment plan. The case manager then works with the individual and the individual’s family or significant others to accomplish the treatment goals.
FEES: State grants and Medicaid reimburse these services for indigent individuals with a serious and persistent mental disorder. Otherwise, fees are adjustable based on a sliding fee scale according to income and number of dependents living in the home.
ACCESS/REFERRAL: Adult residents of Cherokee County Kansas that meet Kansas state criteria for serious and persistent mental disorders (see page 15) can access case management services through their licensed professional therapist and treatment team.
